Accommodated Test Request (ATR)

  • Before submitting an ATR, you must provide your instructor your accommodation letter and connect with them to coordinate your accommodations. If you have not, your instructor will decline your request, as your accommodations are not in effect until this coordination has occurred. If you are aware of any potential scheduling conflicts (e.g. back-to-back classes that may necessitate scheduling the exam for the first class prior to the class exam time), please discuss alternative before submitting an ATR.
  • In order to schedule an exam (in-class or online timed assignment, quiz, test, or exam) with the DRC testing office, please enter your UFID and complete the following request form.
  • You must initiate this process at least four (4) business days prior to your exam. Business days DO NOT include weekends.
  • You will not be able to submit requests past this date. If you miss the deadline to schedule the exam with the DRC testing office, you may have to take the exam with your class without accommodations.
  • Be prepared to provide the following information, which can be found on your course syllabus or from your instructor:
    • Course and section number
    • Instructor name, e-mail address, and phone number
    • Exam dates and times
  • Be advised that submitting inaccurate information will cause your request to be declined by your instructor and returned for corrections. This could result in a delay in the scheduling of your exam or in your exam not being scheduled with the DRC testing office.
  • Requests to modify an exam (date or time) must be approved and submitted to by your instructor not less than two (2) business days before the exam date.
  • If you are taking an online assessment, through Canvas and ProctorU, you do not need to complete an ATR unless you would like to schedule your exam at the DRC.
